Annual Membership Plans

We know not everyone has insurance and we feel quite strongly that this should not prevent patients from receiving essential preventative care. To address this, Dr. Fields and our South Tulsa Smiles team offers an in-house annual membership plan to keep your smile healthy without breaking the bank. This ensures the whole family can get quality dental care to prevent more serious and expensive issues down the line. Here’s what our membership plan includes:

Annual Fees:

• Single Members – $349 per year
• Spouse/Partner/Dependents – $299 per year

Annual Benefits:

• 2 routine exams
• 1 set of routine x-rays
• 2 regular cleanings (deep cleanings will be discounted)
• 2 fluoride applications for kids under 16 years old
• One emergency exam and x-ray
• Eligible treatments discounted at a rate of 10%
